We provide individualized, person-centered support that empowers individuals and families to reach their goals, build independence, and fully participate in their communities. We are passionate advocates for people with Intellectual and Developmental Disabilities (IDD) and other underserved populations.

Our work is grounded in Person-Centered Thinking, Person-Centered Planning, and Home and Community-Based Services (HCBS) principles. We listen first, honor each person's choices, and build support around the life they want, addressing the everyday factors that shape health and well-being.

Abraham Health Services was founded on a simple belief: every person deserves support built around their own goals, not squeezed into a one-size-fits-all program.

We bring extensive experience across healthcare, social service, and community-based systems, helping individuals and families navigate complex programs and connect to the resources they need to thrive. Our team has helped people establish healthcare, access food and nutrition programs, secure emergency, transitional, and permanent housing, and link to county, state, federal, and community resources, always working to address the everyday factors that shape health and well-being.

Our leadership combines advanced education in Healthcare Management and Business Administration with hands-on experience in advocacy, case resolution, complex care coordination, and community engagement, including managing high-priority cases, collaborating with healthcare systems and government agencies, and supporting high-level case escalations. We pair that depth with strong partnerships across community organizations, healthcare providers, managed care, advocacy groups, faith-based organizations, housing providers, and public agencies.
